Welcome to the latest episode of ACCESS, the official podcast of the SVIN Journal.

The May issue highlights a broad range of advances across neurointervention from ultra late time endothrombectomy, strategies to manage tandem occlusions in stroke thrombectomy, to durability of flow diversion for aneurysm treatment.

Given the increasing utilization of mechanical thrombectomy, Levin, Carrion and authors investigate the common medical complications in acute ischemic stroke hospitalizations in the United States between 2010 to 2022 after intra-arterial therapy.

Analyzing the national inpatient sample data, the authors find that in-hospital mortality over time has declined while acute renal failure and deep venous thrombosis have increased.

While carotid webs have increasingly been recognized as a potential risk factor for ischemic stroke, posterior circulation arterial webs are less well reported.

Green and authors report on a single center experience with a literature review and identify a total of 26 cases of webs involving the vertebral, basilar or subclavian arteries.

The lesions are similar to carotid webs with consistent shelf-like intraluminal morphology, suggesting a single disease spectrum.

Ongoing clinical trials are focused on the use of this device for the management of hydrocephalus.

Malik and colleagues describe a case of medically refractory and rapidly clinically progressing idiopathic intracranial hypertension managed with the E-Shunt placement as compassionate use for the recovery of vision.

This promising new approach may have important implications for both acute and chronic elevated intracranial pressure.

The authors outline key technological and clinical trends shaping the next phase of stroke and cerebrovascular care, emphasizing integration of devices, data and systems of care.

Cover art features a detailed look at the microarchitecture of intracranial clots from contributors from the University of Iowa.

In this month's podcast, we continue our series featuring innovators in the field.

They discuss her journey from neurointervention to mobile stroke unit implementation to her focus on systems of care solutions.
