What are the most common mistakes in psychopharmacology, and where would clinicians benefit from improvement in these areas?

I think the number one mistake is when we forget to refill our patients' medications and they get mad at us.

But outside of that, I think sometimes we dose the wrong medicine for the wrong condition.

I think we dose drugs too low, and then we don't see the outcomes or the effect we want.

Sometimes we push medications too high or mix too many medicines together and-

They're common everyday things because we're doing these things every day with our patients.
