In June, OpenAI was training an experimental model that was never meant for the public.
One of its practice tasks was a research question about government spending on skin condition medicines in the Australian state of Victoria.
It found a way into a Medicare statistics website run by Services Australia and the federal agency that delivers Medicare, the country's public health insurance.
OpenAI says the model ran commands and pulled out internal files and credentials.
The notice went to a public email address, the one researchers write to when they spot a security hole.
Australia's Minister for Government Services later explained how that inbox works.
So a warning about an AI agent inside a government system sat in the same pile as the pranks.
Staff opened it the next day, checking it took a couple of days with a weekend in between.
The Australian Signals Directorate, the government's cyber defense agency, heard by the 15th.
