This is a 69-year-old woman who was referred to me just because of ongoing fatigue and sense of pain and difficulty to lose weight.

Now, she herself did not know that she had type 2 diabetes, but as I was reviewing her records from 2023, 2024, and the recent labs from 2025, which were annual blood tests, her A1c was trending upwards.

She was at 6.5%, which is an A1c that meets the criteria for type 2 diabetes, and then it started encroaching towards 7, 7.2%.

And she's completely oblivious or was not aware and did not have these discussions at her usual clinical care, wherever she was getting her care from.

Now, the first thing was, while I'm listening to her, she had this ongoing neuropathy because of a lumbar spine degenerative disease for which she was already on longstanding use of opioids and was seeing a pain specialist.

She was first informed during that visit, while I'm understanding her story, her history, her clinical evaluation, reviewing her labs, that, hey, did you know that you've been dealing with type 2 diabetes for probably two years or even more, which you're completely unaware of? And she was in a state of shock that she was not aware that she was even dealing with high blood glucose.

Now, the reason I'm sharing this story is also to help you understand that there is so much that we can do by just getting empowered with the right knowledge.

And then rather than buying fear and anxiety with a certain number, that is our A1C or the weight, what can we do next so that I prevent complications or I don't let this condition get worsened or can I control it? And also, what are the ways in my lifestyle, which is holistic, comprehensive, that I can work on towards improving my health? This is what I call a true patient-physician discussion, shared decision-making, and also the value of time.
