This was about twenty different AI scribe vendors, all approved for physician use and tested against simulated patient encounters, with actual clinicians comparing what was said against what got written.
They added things to the plan that never happened, referrals for therapy, blood tests ordered.
Twelve of the twenty recorded a different drug than the one the doctor prescribed and stated in the visit, and seventeen of twenty missed important details about the patient's mental health, even though those details were spoken out loud in the recording on purpose because it was a test.
On the procurement side, eleven of those twenty vendors submitted no third-party audit reports at all.
So somewhere there is a government office where did not provide evidence that the product works is an answer on an application that still clears the approval test.
After the report came out, the auditor general mentioned, almost in passing, that she'd recently gone to see her own doctor, and she noticed the office was using an AI scribe.
So she said to him, "Please look at the transcript when you're done." The woman who audited these systems went to her own appointment and asked her doctor to double-check the note.
