know money is the obvious one that comes up here that i talk about but is there a sense of satisfaction is there a sense of purpose that you get are there other intangible rewards that you derive from your work and then also this sense of commitment or this is where you really subscribe to the the ideals of medicine you see that the mission of what it is of why you're doing and you're willing to go above and beyond And this also falls into things like a sense of growth and trying to stretch yourself as well.

What makes that transition so challenging for registrars?

I think it's talked about as a transition by many people, but increasingly, the more that I've worked with GPs, it's apparent to me it's not so much the transition as an adjustment.

It's quite stark, the change that we see going from working in a hospital setting to working in community.

The registrars, when they're making that jump, it can literally be from one day to the next, that they've grown up effectively as a doctor in the hospital system, and now all of a sudden, they are in the room alone by themselves with the patient.

And I've had registrars in my research, you know, say to me they, they sit in the room and they kind of look around when the patient's there and go, "Oh, hang on, I'm the doctor.

I'm the one that's got to make the decisions here," and have that sense of responsibility that's on them.

The lack of team sharing of responsibility, that sense of isolation can be quite profound.
