I'd really like to ask you, Ralph, in your experience, from a primary care perspective, what happens in that gap?

Again, you're asking the tough questions, but you're right.

The communication between hospitals on discharge into the primary care settings needs to be improved.

Um, we don't have a really great, um, clinical handover.

I mean, there are some key initiatives that are happening in some of our regions, like Queensland, that are really making a difference.

But, you know, as you said, if people are discharged from hospital and they're not on guideline-directed medical therapy, the first thing the GP's gonna ask is, "Well, why didn't they start that tablet in hospital? There must be a reason for them not to start it." So that communication about when to titrate after they've been admitted I think is really clear.

And yet the evidence behind that early titration is just fantastic.
