I, I'm curious if we can get a little bit more granular on how... like, how do you actually begin the transition? What are these conversations like, and what sort of tools do you work on with the patients to get them ready for booking and keeping appointments, keeping track of their medications and filling scripts, getting their blood work done? Some of the basics of attending adult appointments that, in the absence of a multidisciplinary team, we may not have the resources to remind them about or coach them on in the same way.

So there are different models that exist, and right now we don't have evidence that one model is any better than the other.

I think just the concept of having a transition care model at your center is excellent.

So I can explain our experience here at McMaster, and again, there are multiple other multidisciplinary transition clinics across the country that do wonderful work.

So at McMaster, we start at the age of 14, and any one of our patients who we anticipate needing adult rheumatology care will be booked at least once a year in our combined transition clinic.

So this is a multidisciplinary clinic that runs twice a month with two different adult providers.

In this clinic, they meet with the new adult rheumatologist, as well as their regular pediatric rheumatologist.
