Hi, everyone, and thank you for joining us today.

I'm glad to be here with the SIREN community to share a journey we have been on for several years.

From kind of a health communities model to a regional health hubs, we are now building in New Jersey.

Over the next few minutes, I want to walk you through three things.

the lessons we learned about implementation, what the data actually showed us, and how those HC experiences shape the regional health model we are implementing today.
