And so where are these costs compounding and what really is kind of the misalignment there? between people getting quality care and that cost of care being so exponential that it is not really sustainable for individuals or for employers.

Really, when costs are uncontrolled and where they enter the health plan, that's really the first place where things start going off off the rails.

And so for the approximately 100 million self-funded employees that exist across the country, what that means is that where specialty care in particular, which is about 55% to 65% of the reason why the premiums go up is due to specialty costs compounding, the real issue is where those costs begin, the site of entry.

And so for hospitals and for those who are consuming emergency room services, those things, whether you're going as specialist for an orthopedic surgeon, as an alternative, if you start off with more conservative things like physical therapy, for example, Those are two very different cost paths that compound over time.

One might end up with a musculoskeletal issue that can be wrapped up through bundles in an ambulatory surgery center outside of the hospital setting that might be $15,000.
