John, can I ask you a little bit about bronchiectasis and immunodeficiency and the mechanisms behind the airway damage? Do we know how much of it is because of infection and how much of it might be due to inflammation and airway remodeling?

Answered, I suppose, in the 60s by Peter Cole, who was a clinician at the Brompton, who came up with this idea of a vicious cycle in bronchiectasis.

So the idea that you've got impaired mucociliary clearance, bacterial colonization, and airway inflammation.

So, for example, cystic fibrosis is a cause of bronchiectasis, but is principally there because of the impaired mucociliary clearance, at least at the start.

So then talking about prevention being better than cure and longer term risks, at what point should clinicians start thinking about advanced respiratory support, pulmonary rehabilitation, oxygen, transplant referral, maybe in people who have inborn error of immunity associated lung disease?

I think the main bit with the sponsors is that we, well, I look at it from when I inherited the commercial role and sponsors were not engaged.

and do you know what the professionalizing ways of working so look there is a contract and it's black and white what's a part of that contract but actually you know if there's a problem just pick up the phone or come and see us or i'll go and see that particular person not that there are issues but you know it's being that face of the club out there that people know that they can have a conversation with and kind of nothing's off the table so I think the approachability of the club and just opening our arms and, you know, making sponsors feel valued.
