What do you think with this patient given it's penetrating trauma? Do we deviate or do we really run a traumatic arrest protocol?

I think trying to deviate from protocols in times of stress always adds a whole extra layer of complication.

And I think it's particularly hard in Australia because we're so used to what I would refer to as low-velocity penetrating trauma, so stab wounds particularly.

But gunshot wounds as a high-velocity injury have so much more scope for injuries that we don't see at skin level.

And so...

My approach would be that it's safest just to run the full traumatic cardiac arrest protocol

Do you have a better understanding,

Jess? So my understanding is that the international guidelines are for extra thoracic penetrating trauma, so anything outside of the chest.

What do you reckon Jess?

So I think the important part of this as far as I would think would be making sure you've got lots of plans.

This is not a patient where you've got one size fits all and you're going to be able to get an immediate line in and fix your problem.

This is going to be a difficult patient and so you really need a plan A, B and C in this scenario.

I think that requires lots of team members.

We've got plenty of people there.

Any problems with rick lines in your experience, Jess?
