For our listeners who may not routinely manage pathologic myopia, can you discuss what that is and kind of what lacquer cracks are and why they matter clinically?

So lacquer cracks are breaks in the RPE, Brooks membrane, and choriocapillaries complex.

They incur in, as you said, pathologic myopia.

As the eye elongates, especially axially, Laplace's law kind of comes into play and tells us that with increasing globe radius, we have increasing wall stress as well.

And over time, that mechanical stretch can produce some of these focal ruptures.

Once a lacquer crack actually forms and matures, the diagnosis is usually pretty straightforward because it has pretty well-established imaging features, particularly the hyperreflectivity on the near IR and the hypofluorescence on the IgG.
