Because the new nail is engineered to bear more weight than earlier versions, patients can be on their feet in a matter of weeks as opposed to months, and better outcomes may make this procedure more popular.
15:25
We're not making better basketball players.
15:27
We are making happy people.

... also helped design the proprietary technology used in the procedure.
15:39
One shouldn't take this frivolously.
15:41
It is a process to go through.
15:44
And your muscles could become stiffer.
15:45
Your joints become stiffer.

So for anybody who's just tuning in, Dr. Bailey, can you reiterate what the weight-bearing capacity is for each nail size? I know it varies by the patient's body mass and other factors, but this is a common question that keeps popping up.
3:16
I'll start by telling you what the FDA says.
3:20
Okay.
3:21
So the FDA guideline that the nail was approved under has a flat 50 pounds weight bearing allowed.
3:30
What's interesting is their guideline for a 10 millimeter nail, a 11.5 millimeter nail, a 13 millimeter nail, and a 14 millimeter nail diameter is 50 pounds for each of them.
3:46
Which is, I think it's a reaction and it's what they wanted to stay on the super safe side.
3:56
that we are using it, then let's call it off-label, those guidelines.

Our
3:17
So can you walk us through what exactly that means on a day to day basis, you know, during lengthening, during consolidation and so on?
3:23
Well, first of all, I want to state a disclaimer, and that is that the FDA does not recommend this nail for full weight bearing.
3:32
I think a lot of that probably dates back to some of the issues, not with weight bearing, but just some of the issues with the previous weight-bearing nail, the stride nail.
3:41
And so they have...
3:44
So this nail is approved for more weight-bearing, but it is a more conservative recommendation than we recommend at the Paley Institute.
3:54
Our protocol, and anybody who knows anything about...
3:58
the paleo institute limb length stature lengthening program knows that we are protocol driven and what does that mean protocol driven means that we follow a recipe a tried and true recipe that works over and over and over to give the best possible results and once we've figured out that recipe we don't deviate from that we don't make exceptions we don't deviate from it and the reason is Because we're achieving results that very few centers, if any, are achieving.

Okay.

And so they, they have the, so this nail is approved for more weight bearing, but it is a more conservative recommendation than we recommend at the Paley Institute.
4:13
So if you want to call it that, we are, our protocol, and anybody who knows anything about the Paley Institute limb length, stature lengthening program knows that we are protocol driven.
4:30
And what does that mean? Protocol-driven means that we follow a recipe, a tried and true recipe that works over and over and over to give the best possible results.
4:43
And once we've figured out that recipe, we don't deviate from that.

24 MINS LATER

would that be reduced with the pmax for patients who are lengthening let's say up to five centimeters or so
