What is the recommendation for younger fit patients?
3:45
So in advanced stage classic Hodgkin lymphoma in younger patients, the recommendation is four or six cycles of BRICAD, a BCOP variant, including printaximab-vidotin, based on the PET result after two cycles.
3:57
Patients with a negative PET after two cycles receive four cycles, and those with a positive PET, six cycles, optionally followed by a PET-guided radiotherapy.
4:05
And this is based on the results of the HD21 trial that investigated this approach with BRICAD against the same approach with a former standard consisting of escalated BRCOP and with BRECAT, the PFS results were not only non-inferior but also superior than with escalated BRCOP and the protocol was associated with a reduced rate of toxicities such as infertility.
4:29
So this protocol is now the new recommendation for younger patients with advanced stage classic hodgkin lymphoma.

If you want our audience to remember one thing from this podcast, what is that?
10:22
Actually, the integration of these targeted agents that Igor mentioned, brentuximab, vedotin, and the checkpoint inhibitors, improve treatment and, at least in the short term, reduce toxicity.
10:33
And again, we do not yet know what checkpoint inhibition makes with patients in 20 or 30 years.
