David, from your perspective out at the Minneapolis VA Medical Center, I'm just wondering, like, could you describe maybe some of these patterns or the gaps, right, that are revealed during the process? And then how do you then take those gaps? How do you identify them? But then how do you turn them into opportunities for prevention?
33:01
Yeah, it's a great question, and I think that's where collectively we're all doing similar work and putting our heads in the same space and voices in the same space help us identify these gaps.
33:12
Oftentimes we're seeing these transitions of care where a veteran might be leaving our health care system and going into the community or working with a nonprofit, and we kind of say goodbye, and there isn't that follow-through.
33:23
And this process allows us to look at how do we strengthen that? How do we support the veteran during care transitions or these care pathways? to make sure that they have that support, that we're connecting with the family, we're connecting with the kids if there's family support as well, to make sure that there isn't a start and end, that it's a continual transition regardless of the system at the VA or in the community.
33:46
And there's always different gaps, whether it's means restriction.
33:50
You know, there's a lot of different gaps that we look at where veterans just didn't even know that the service or opportunity existed.
33:57
And so as we ask the questions or look at how come this question wasn't asked, we can start changing our programs and our systems in place to start intervening.
36:59
I don't know.
