Sometimes I feel we're always on the bookends of that, not letting a therapy be tried long enough at optimal dosing or waiting so long past the point of efficacy that the patient's extremely deconditioned or very advanced by the time they seek out alternative therapies.

And I think it's because in a busy clinical practice, it's not always easy to obtain repeat bone marrow assessments or perhaps if they're receiving growth factors, they're going along and not always seeing the provider.
