78: I’ll Take My Burnout To Go, Extra Cheese

You took a vacation, a real one, a full week, maybe two.

You slept.

You saw people who love you.

You did the thing everybody tells you to do, and you did it correctly.

And on the last night, driving home or sitting in the airport, you felt it come back.

Not the tiredness.

The tiredness was gone.

Something underneath it that had been waiting the whole time, and you thought, "The vacation worked, so why do I still not wanna go back?" Here's the answer.

Rest fixes tiredness.

We use one word for all of it, burnout.

It's become the container we throw everything into.

But burnout is a specific thing.

It's emotional exhaustion, depersonalization, depletion.

It means the tank is empty, and the treatment for an empty tank is filling it.

Rest, time off, reduced load.

That's why when burnout is genuinely what's wrong, the vacation does work.

You come back different.

There's a second thing, and it's what we've been mislabeling for about twenty years, and I've mentioned it before.

Here's how the AMA defines it.

The experience of knowing the right course of action for a patient but being unable to act because of external constraints, such as policies, laws, administrative barriers, or broader systemic issues that stand between physicians and the care our patients need.

That's moral distress.

Moral distress is the experience.

Moral injury is what accumulates when you experience it long enough.

It's about being placed in a position over and over where you know what the right thing is, and you cannot do it.

And here's a distinction that matters.
