In transplant-ineligible patients, based on the ECHO study, acalabrutinib plus bendamustine rituximab is now approved frontline option for our folder, as well as transplant-ineligible patients.

The second subtype where I do use a lot of BTK inhibition is Waldenstrom's macrogablanemia.

BTK inhibition is an established treatment strategy, particularly useful when we want an oral chemotherapy-free approach.

Then there's marginal zone lymphoma in the relapse setting for patients who have had experience prior anti-CD20 antibody treatment based on the Magnolia study.

In my high-risk subset, I know that that may not be working for too long.

So I'm planning the next steps like getting them to CD19 CAR T-cell therapy or a clinical trial.

In Waldenstrom's marginal zone follicular lymphoma, I think the next treatment on progression really depends on the duration of prior benefit, the deceased tempo, refractoriness to prior treatment, or is it just intolerance, patient frailty, and then available clinical trials.
