EP 136: The Otolith Organs: The Most Underassessed Structure in Concussion Care

In Canada, I don't know about the US, but... or the rest of the world, but in Canada, it takes months and months and months to get into an ENT.

So but there have been multiple studies that when they do measure those devices or those outputs, have shown that otolith dys- dysfunction in MTBI using VEMP testing with rates of abnormal findings significantly higher than would be expected otherwise, ultimately, okay? Um, yep.

[chuckles] Excuse me.

I, like, have notes, and then I start to read them, and I'm like, "I don't wanna read that part anymore." Um, so what does this actually look like clinically? What are some of the things that our clients might be saying that we can associate or attribute to a potential otolithic dysfunction? You guys, once you see it, you can't not see it, okay? So dizziness or imbalance from linear movement rather than rotational movement.

There was a study that was done that looked at subjective visual vertical.
N
17:13Natasha WilchHOST
If they had a subjective visual vertical greater than two degrees from vertical, that correlated to worse blood pressure outcomes compared to a normative group.
N
17:23Natasha WilchHOST
What? Isn't that cool? And then there was another study that looked at, uh, VEMP testing, uh, and abnormal VEMP testing and what that showed in blood pressures too.
N

Same thing.

How well do you know where you're at? There's other things beyond that that we'll talk about too.
S

so first though right i showed you the testing with the goggles that is eye movements super super important much more important than you would ever think and people usually are symptomatic when they do it but outside of it most people think they do well and they are shocked to see their videos because 90 roughly are struggling even though they think they did well positional testing which we already talked about caloric testing putting stuff in the ear rotary and then you have some of these other testings, your VEMPs for otoliths between the two.

Now, everyone wants to know, well, what do you do with this? Well, this is ultimately where we do therapy to rehab the brain.

Now, a lot of people are being sent to generic standard vestibular therapy, and this is no knock on them, right? They're doing the best they can, but I do treat a lot of people who are physical therapists, OTs, nurses, and everything else in the health field.
