Troponin

A common pitfall will be with troponin testing.

So a one-off normal troponin does not rule out ACS, and a raised troponin does not rule in ACS.

Troponins can be raised for a number of reasons, such as chronic kidney disease or acute kidney injury, underlying infection, or heart failure.

So it's important that we do serial troponin testing to see, are they improving? Are they further elevating, or are they static? Once you've gone through your ADE, it's important then to reassess.

So we've given them analgesia.

The BNP was 41.

We draw blood and look for a specific cardiac biomarker called troponin.

Troponin is a structural protein locked deep inside the heart muscle cells.

It is only released into the bloodstream if the cell membrane physically bursts open and dies.

They compared transvenous ICD implantation with defibrillation testing, transvenous devices without testing, and subcutaneous ICD implantation with testing.

Troponin was measured before implantation and again afterward.

Within the transvenous group, the investigators also asked whether more total shock energy tracked with a larger biomarker rise.

The main result was not just that troponin increased.
