DRESS is a potentially life-threatening drug hypersensitivity reaction, and systemic steroids remain first-line treatment.

But prolonged steroid courses can come with significant toxicity, and patients may relapse during tapering.

Researchers therefore looked at whether adding the JAK inhibitor tofacitinib could improve outcomes and reduce steroid exposure.

In this retrospective study of 61 patients, those who received tofacitinib in addition to systemic steroids had faster near-complete rash resolution and a shorter overall steroid course, about 58 days compared with 88 days with steroids alone.

Tofacitinib was also generally well-tolerated with no serious adverse events reported in the treatment group.

The takeaway is that adjunctive tofacitinib may offer a promising steroid-sparing strategy for select patients with DRESS, potentially speeding skin recovery while reducing prolonged steroid exposure.
