Lord Toby Harris is the chair of the National Preparedness Commission and he's seriously concerned about the organisation's ability to manage the UK's airspace.

Any organisation has to work on the assumption that some of their systems may fail.

It might be because of a cyber attack, it might be because of a software glitch, but you can't just work on the basis that it's always going to work every time.
