If you're a practitioner sitting across from a patient who is utterly terrified of germs, experiencing a racing heart, sweating, sheer panic at the sight of a public restroom, isn't that just a severe anxiety disorder? Why did the psychiatric community feel compelled to evict OCD from the anxiety category and build it an entirely new diagnostic framework?

That's the perfect question, and the distinction lies entirely in the underlying neurocognitive mechanism.

Anxiety disorders are fundamentally disorders of fear processing.

Or the somatic terror of panic attacks.

The fear circuitry, heavily reliant on the amygdala, is overactive in response to those external cues.

OCD, however, is driven by internally generated intrusive thoughts.

So the threat is coming from inside the house, so to speak.
