He's mixing his bass tone, his mid-range tone, all together as he's singing the verses.

The song was originally co-written by musician Susanna Melvoin with William Orbit and she revealed that she wrote the phrase Candy Perfume Girl while grieving the tragic death of her brother Jonathan who was the touring keyboardist for the Smashing Pumpkins.

He died of a heroin overdose in 1996 and Susannah confirmed that the track was personal for her while she was grieving.

When Madonna joined the project she was able to connect with the emotional energy behind Susanna's original idea of the track and kept her original framework then shaped it into a broader metaphor about destructive desire.
