Splenectomy

So we see patients with splenic malignancies.

Splenectomy is not typically... indicated for a diagnosis of lymphoma.

But there are some patients that it can be therapeutic for, such as splenic marginals on lymphoma.

Oftentimes, these patients have splenomegaly.

I don't know if all people will have a lot of failures as I did.

Splenectomy is always on the table as an option.

to help treat this disease.

I've tried to keep that as a last resort.

BTK inhibitors in relapse if needed, and watchful waiting is a genuinely respectable active surveillance option throughout.

Splenectomy is now second line, although often happens up front when you're trying to diagnose splenic marginal zone in the first place.

And if you're going to take someone's spleen out, don't forget their spleen precautions, vaccination, antibiotic prophylaxis, and education.

And then just watch out for that 5% to 10% of patients that will transform to a large cell lymphoma.
