So this creates a completely different rehabilitation environment for the patient.

Now, when it comes to neuropathy, so imagine wearing a patient wearing a prosthetic sock that is creating excessive pressure, right? A normal person with normal sensation, like with no neuropathy and protective sensation intact, because people say it hurts, right? But if the patient has neuropathy and they have that excessive pressure from a prosthetic, right? Or a prosthetic sock, the pressure can continue and the skin can become irritated, a blister can form and a blister can become an ulcer and the ulcer can become infected, God forbid, and suddenly the residual limb management problem becomes a wound care problem, okay? So this is why prosthetic fit plus skin inspection as well as sensory status matters.

You need to consider that.

Now, when it comes to perfusion, right, we already know we love us some vascular.

But what happens when the limb shrinks after the prosthesis? has been already fitted.

The socket may suddenly become loose and the patient may experience what we call pistoning, rotation, instability, increased pressure, shear, skin irritation, and changes in gait.

And the patient may need may begin adding socks even don't do that don't ever self-treat okay the prosthesis may have to modify the socket the linear interface may have to change the socket may eventually need a replacement and this is why residual limb volume management continues beyond the first prosthetic fitting now we love us a prosthesis we love you and this is where they're extremely essential okay This is not just somebody making an artificial part or a limb, you know, or an artificial leg, right? They're helping managing the interface between the human tissue and the mechanical force.

So they're evaluating the limb shape, the volume, the skin condition, the gait, the alignment, the interface materials, the suspension, the soft tissue distribution, the bony promises, and I believe I already said skin condition.

It deals with veterans who have come off the front line who have lost a limb.

We are looking at how we can help them build a workshop that will support prosthetic limbs.

It also is designed to have the veterans themselves do the work to sustain it and to help them in their recovery process.

And if you're not aware, Multnomah County has a growing Ukrainian population.

Multnomah County District Attorney Nathan Vasquez says the trip was organized by the Central East Portland Rotary.

One of our members is Ukrainian, and he created a relationship with our counterpart in Chevernitsy, Ukraine.

They helped set up a workshop for war veterans, Prosthetic Limbs.

Gresham's mayor also went to establish a friendship city relationship.

Despite the war, Vasquez says it's a beautiful country.

And so I was like Mr. Prosthetics guy.

And so I didn't like his first nose.

It was too big.

Chapter 14 The New Arrangements as to Remissions Artificial Legs Another Interview with the Visiting Director Compose Verses Hospital Once More Fenians Prisoners' Letters Asking pardon of my readers for the rather serious digressions I have made in the preceding chapter, I now return to my narrative.

Shortly after the new regulations were made known to the prisoners, I wrote a letter to my brother, and in this solitary instance I confess in a somewhat ironical strain, and as a matter of course, the letter was suppressed.

I remember one passage in it was to the following effect.

At the desk sat the new director, by his side stood the governor, and in front of the desk, the chief warder.

"Well, what do you want?" I told him that I had lost my leg in prison, that I was feeling my health giving way, that I was anxious to be in a position to move about a little better, and would feel very grateful if he would allow me to have an artificial leg, the same as the other prisoners had.

The governor endeavored to deny that any artificial legs had been furnished to prisoners.

But being prepared for something of that kind, I gave the particulars I have already mentioned, which were confirmed by the chief warder.
