Oxygen is the most widely used medication in ACLS.

Assessing a patient's O2 saturation using a pulse oximeter and giving supplemental oxygen to patients that meet criteria is an early action in the bradycardia, tachycardia, ACS, stroke, and post-arrest ACLS algorithms.

Knowing when we need to give oxygen and when it isn't necessary based on pulse oximetry is today's PassACLS tip topic.

Today we're going to take a closer look at that last one because VO2max has escaped the exercise physiology laboratory and apparently moved on to everyone's wrist and I'm actually quite pleased people are talking about their cardiovascular fitness.

For years so much of the fitness industry has focused on what our bodies look like, body fat, muscle definition, weight, clothes size and But your ability to take in oxygen, transport it around your body and use it to produce energy tells us something about what your body can actually do.

The problem comes when we turn one useful measurement into one and the only measurement that matters.

So today we're going to understand that number without becoming obsessed by it.

And to do that, we need to start with what VO2 max actually means.

So pop on those trainers or grab that cuppa and let's dive right in.

So what the heck is VO2 max? Let's decode the name first.

The V refers to volume.

The case stayed completely cold for the better part of seven years, and it wasn't until it got national media attention that anything started happening again.

On November 3rd and 4th, 2018, a two-night special called The Disappearance of Phoenix Colden aired on the Oxygen network.

It was built around a fresh investigation by Shondrea Thomas, a St. Louis television journalist, and Joe Delia, a retired deputy police chief.

What they found changed the shape of the case, because the sheltered homeschooled girl of the early coverage turned out to be only half the story.

Further, the lights weren't on and the keys weren't in the ignition.

To Officer Perry, there'd been nothing about the scene to raise a red flag, just an ordinary abandoned truck that looked for all the world like it had run out of gas.

Ultimately, the Oxygen documentary was unable to confirm how this narrative inserted itself into the case.

What was clear is that by then the idling engine narrative had traveled everywhere, even onto the Missouri State Highway Patrol's official missing person bulletin.

Only 24% of the patients didn't require oxygen anymore over the 72-hour study period, and nearly 50% of them actually got to the full 72 hours, so that's pretty good.

I guess the remaining 20% had to have an escalation of their O2 support.

Interestingly, there was actually fewer oxygen titrations in the autonomous group.

Isn't that interesting? And despite that, these patients spent 22% more time in the target zone of 90 to 96%.

Not that Tran or the others ever let that last more than a minute.

There are always cables to tighten, numbers to shout, jokes about how decompressed your brain would be if the O2 mix glitches.

Underneath the bravado is something raw, unease.

And tonight, it shivers through me differently.

It's called ozone.

Three oxygen molecules bound together that has been a game changer for me personally and for thousands of my patients.

The number one international expert on ozone is a friend and mentor of mine, Dr. Frank Schallenberger.

He is a board-certified physician in anti-aging medicine and the U.S. president of the American Association of Ozone Therapy.

Most people who think of ozone think of the ozone layer and sort of the holes in the ozone.

And just explain to the audience out there what ozone is.

So basically ozone is oxygen.

Oxygen is a molecule that doesn't have enough electrons.

Yeah.

So the solution to pollution is dilution.

So when we talk about stuff like this, there was even a study that came out not too long ago that I think you and I talked about, and it was that high CO2 actually causes carbonic acid within a space, and I did a whole deep dive on that just to have Rachel prove me wrong about me not knowing anything about science, which was good.

I learned so much from it.

And it was just that something like that high CO2, when high CO2 dissolves into a liquid like water, can create carbonic acid, but not in a high enough volume to actually cause coil failures.

But what it does, the carbonic acid, is actually wash the surface of protective coatings so that other acids can come in and do their work.

And so you'll find that there's a lot of layers to this whole problem that we're trying to solve, and that is that there's a ton of things that bump into each other and cause other problems along the way.

It's these dominoes that are knocking into each other over time.
