Claire HarrisonMODERATOR
16:35
Hello, my name is Claire Harrison.
Claire HarrisonMODERATOR
16:36
I'm a hematologist from London, UK, and I'm delighted to be here at IWMPN MDS in Washington with my lovely colleagues who are just going to introduce themselves.
Naveen PemmarajuGUEST
16:47
Hi, thank you, Claire.
Naveen PemmarajuGUEST
16:48
Naveen Pemmaraju, professor of leukemia at MD Anderson.

11 MINS LATER

Claire HarrisonMODERATOR
28:16
And then lastly, I think the last agent we discussed in that session was actually a metastat.
Pankit VachhaniGUEST
28:21
Yes.
Claire HarrisonMODERATOR
28:21
Another drug where there's an overlap between MDS and NPN.
Claire HarrisonMODERATOR
28:25
So we're Really intrigued to see now the data that's going to come from this study that we've been running for a really long time, looking at a metal stat, the telomerase inhibitor versus standard of care as a second line.
Mark LewisGUEST
23:22
You know, I've now treated hundreds of patients with PROT.
Mark LewisGUEST
23:27
And I've seen a rate of MDS and even AML of about 1% to 2%.
Mark LewisGUEST
23:32
And those cases are so memorable, right? Because they're exactly what you and I fear the most.
Mark LewisGUEST
23:37
You don't want to take someone who has a more indolent disease biology and give them a more aggressive one.
Aman ChauhanHOST
24:38
In my practice, PRRT, I think, is a very commonly used tool, usually in a second-line state, for most of our midguts and gap nets in general.
Aman ChauhanHOST
24:51
But there are always times where I feel in a younger patient, in a very low-volume disease, where I'm worried about all these long-term problems.
Aman ChauhanHOST
25:01
You know, myelotoxic disease, MDS risk, and for that matter, even long-term renal safeties.
Aman ChauhanHOST
25:09
Should I expose somebody with systemic radiation if we can have other tools, especially with new age targeted therapies and even some chemotherapies that can stabilize the disease for a bit? But at the same time, if I have a patient with a lot of extrahepatic disease, which is lighting up very brightly on dotatate pad, has high SSTR uptake, PRRT is a very relatively low toxicity, at least for acute toxicity, well tolerated and low burden.
Sue StanekGUEST
39:32
when and how and making yourself available.
Stan StanekGUEST
39:35
Yeah, and I have a brother right now that has had MDS, which is a similar type of blood cancer.
Stan StanekGUEST
39:43
And we had been a little bit at odds up until his diagnosis, and now we're very close.
Stan StanekGUEST
39:51
And About
Alex MercerHOST
5:06
For Geron, that ends Imatelstat's push into myelofibrosis through this trial.
Alex MercerHOST
5:11
Rytello's lower-risk MDS business is separate from this result.
Alex MercerHOST

Still in oncology, AbbVie has filed.
Alex MercerHOST
5:20
On October 27th, it submitted a biologics license application, its request for FDA approval, for etentamig, a BCMA Bi-CD3 drug in relapsed or refractory multiple myeloma.
Andrew LaneGUEST
4:16
In separate lines, we were looking at other dependencies in BPDCN cells, and it turns out that BPDCN is also uniformly and very sensitive to BCL2 inhibition with a drug like venetoclax more so than aml sort of across the board more like it is in cll where it's quite sensitive to levels of venetoclax that are achieved in patients that was the genesis of combining these three drugs into a triplet regimen it also helps of course that we are leukemia doctors who mostly take care of these patients and we're very familiar with giving venetoclax and azacytidine as we do for patients now as an approved regimen for aml That was the reason that we brought these together into a triplet.
Andrew LaneGUEST
4:58
Finally, I'll say that we did a phase 1b study that preceded this paper published in 2024, where we did dose escalation first of the Tegraxifus azacitidine doublet combination, and then adding in the triplet Tegraxifus azacitidine venetoclax in patients with AML and high-risk MDS.
Andrew LaneGUEST
5:18
That allowed us to come up with the recommended phase 2 dose that was used in this paper.
Laurie SehnHOST
5:23
Can you summarize this phase two study design along with your major findings?
Andrew LaneGUEST
10:03
We have been studying the genomics of BPD-CN, and maybe as we get larger numbers of patients, we will be able to break down prognostic and predictive subgroups.
Andrew LaneGUEST
10:13
But in this, we really weren't able to.
Andrew LaneGUEST
10:15
But I do think it is worth pointing out what you mentioned is that 20 to 30% of patients, and that's exactly what we saw in this study with BPD-CN, have either a pre-existing or a concomitant other heme malignancy, most frequently myeloid disorders like MDS or CMML.
Andrew LaneGUEST
10:32
And it's important that we don't exclude those patients from trials.

The combination lowered the relative risk of radiographic progression or death with a hazard ratio of 0.48.
Maya PatelHOST
5:19
Overall survival is still immature, and the label warns of myelosuppression and of MDS and AML.
Alex MercerHOST
5:26
The combination has been approved since June 2023 for HRR gene-mutated metastatic castration-resistant prostate cancer.
Alex MercerHOST
5:35
This approval moves it earlier, and at that stage, eligibility is set by an HRR test result.
BasharHOST
13:46
That's right.
BasharHOST
13:47
Actually, I'm going to pull up my notes on that because, yes, yeah, because this is so standard with a lot of Dell 5Q in AML and MDS.
BasharHOST
13:54
And the Dell 5Q is good in MDS but bad in AML because it's there with the CP53.
BasharHOST
14:00
I don't know.
BasharHOST
14:01
We need to get an electrical person.
