Moyamoya children with Down syndrome were far more likely to present with an actual stroke rather than warning symptoms like a TIA or headache, 71% versus 21% in the non-Down syndrome Moyamoya group, which works out to an adjusted odds ratio of roughly 14.

They also got to medical care later with longer delays, both from symptom onset to presentation and from the time of first presentation to getting a confirmed diagnosis by vascular imaging.

Interestingly, the delay seemed driven less by access issues and more by recognition failure or diagnostic overshadowing, where early neurological symptoms got chalked up to a child's baseline developmental profile or other common Down syndrome conditions.
