We need vasculature, obviously, to bring all the different types of cells, to bring growth factor, to bring our immune system down, because the immune system not only fights infection, but the immune system is critical for healing, right? It helps to convert an inflamed wound into that proliferative phase of wound healing and move that along the wound healing cascade.

Today, we're discussing a contemporary, multi-center, randomized controlled trial by Lenzer-Link and colleagues published in the Journal of Vascular Surgery in 2026, examining whether closed incisional negative pressure wound therapy reduces groin wound complications compared with standard surgical dressings following open peripheral arterial vascular surgery.

Groin wound complications remain a significant problem after vascular reconstruction, with reported rates ranging from approximately 10% to 34%.

These complications include surgical site infection, seroma, wound necrosis, hematoma, and dehiscence, and are particularly concerning in vascular patients because of their associated morbidity, potential need for re-intervention, and risk of infection involving underlying prosthetic graft material.
