So in a healthy system, cytokines act as these little messengers that bind to cell receptors, alerting the immune system to actual threats.

Confirming a local site is actually actively screening patients is essential before mentally committing to the journey.

So what does this all mean? When we zoom out from the specific FVSI scores and the JAK-STAT pathways, the overarching narrative in these sources is that clinical trials are the ultimate engine of medical progress.

So there are several aspects of myelofibrosis that plague patients.

It is an inflammatory disease as a consequence of JAK-STAT pathway activation.

So fevers, night sweats, weight loss.

Therefore, symptom improvement, which we'll talk about, is a goal of therapy and an endpoint for clinical trials.

Are the molecular underpinnings of these three the same, or there are differences between these three?

No, there are differences, but then there is also a unifying theme, right? So PV is almost entirely driven by JAK2 mutations, whereas ET and PMF are about half to two-thirds JAK2, but about a quarter to a third CALR, uh, and then a, and then a little bit of MPL, uh, uh, which is the thrombopoietin receptor, right? So, so, but, but the unifying theme is that all of these mutations activate the JAK-STAT pathway.

A- and so that has led to the success of JAK inhibitors, you know, you could say, across these diseases, you know, uh, and in a, in a mutation-agnostic manner.

But now I think we're in an era where there's a lot of interest in selectively targeting the driver i-in each condition, you know, with these next generation JAK inhibitors, mutant CALR antibodies, et cetera.

The Journal of the American Academy of Dermatology gives us a paired review of alopecia areata that together amount to a current management framework.

Maas and colleagues cover evaluation and pathogenesis, reminding us that the condition affects roughly 2% of people globally, that tricoscopy findings such as black dot and exclamation point hairs should be documented, and that pathogenesis centers on collapse of follicular immune privilege with cytotoxic CD8 T-cell inflammation, with genetic and pharmacologic evidence pointing to the JAK-STAT pathway and a Th2 contribution in some patients.
