Human error

Who does this happen to? Is it random? Or is it certain people who are, at the end of the day, cutting a corner that causes it? You say a lot of it's preventable.

I assume that means there's a lot of human error involved, obviously.

Yeah.

from my past life, just seeing all these brokers making mistakes, like a lot of people that have agents that, I don't know, don't, don't work in the United States and they've never met him before.

I think the problem is incompetence.

The problem is human error.

It's not human conspiracy, it's human error.

And then it's the powers that be trying to massage those errors or cover up those errors or make those errors go away like Deflategate and Brady and the Patriots because they were the marquee team in the league, but everybody knew they were cheating.

And how do you massage that? Michigan's been now at the heart of a couple of scandals, but that's Michigan.

Um, but the majority of the time, you know, critical incidents occur not because you have, you know, a patient with a congenital heart condition that was undetected or, you know, it, it's not usually the black swan case.

You know, more than seventy percent of the time, critical incidents occur due to human error, and that's the interesting part because it means that we can really improve.

We can improve these outcomes if we just improve the area that we're making errors in.

And say a little bit about, uh, just for folks who aren't familiar with kind of the lingo, when we say human error, what does that mean? Give me an example of what, what is a human error, as opposed to any other kind.

So human error-- Yeah.

So human error would be, you know, um, we'll get a little bit more into this, but making, um, a misdiagnosis, um, maybe not managing the airway as well as you could have, becoming super fixated on one aspect of the clinical picture while ignoring other aspects of or other signs and symptoms that may be pointing to something else.

Um, these are all the kind of mistakes that humans can make.

You know, it's, it's very stressful for us, and unfortunately, um, stress acutely deg-degrades performance, memory, [chuckles] and, um, the tr-- The truth is, you're not gonna be at your pe- your best in a crisis.
