Okay.
A.W. MartinHOST
2:28
If you want, what do they call it? HOMA, IR, which is an insulin resistant test.
A.W. MartinHOST
2:36
They're resistant to do it.
A.W. MartinHOST
2:38
They're resistant.
Sheri BullockHOST
47:31
We have a tip for our listeners, and this comes from the UK, and it's from Tasha.
Sheri BullockHOST
47:37
She said, I know that your US listeners are always sharing their fasting insulin, glucose, and HOMA-IR scores, which is harder for us to get a hold of in the UK.
Sheri BullockHOST
47:47
I just wanted to share that Dr. Chatterjee, who has a podcast called Feel Better Live More, has developed a health app where you can get important blood markers tested in the UK.
Sheri BullockHOST
47:58
His tests include fasting glucose, fasting insulin, hemoglobin A1C, and a HOMA-IR score.
Sheri BullockHOST
48:05
You pay for a year and then the labs are repeated three times over that year so that you can see if you are making improvements.
Sheri BullockHOST
48:12
I know our measurements are different in the UK to the US, but I think HOMA-IR is the same and mine came back at 0.5.
Sheri BullockHOST
48:22
So I can see that fasting is definitely make a difference to me as when I had my hysterectomy three years ago, they had found the start of a fatty liver.
Sheri BullockHOST
48:30
I can see the changes I've been making since then have been helping.
Matthew BernsteinGUEST
44:55
We're doing the advanced lipid profile, which is the NMR fractionation of all the different lipids.
Matthew BernsteinGUEST
45:04
We do the HOMA-IR test, hemoglobin A1C, obviously.
Matthew BernsteinGUEST
45:09
We're getting...
Matthew BernsteinGUEST
45:10
You know, obviously like a CBC, we get a homocysteine, HSCRP, iron levels, vitamin D levels, copper, ceruloplasmin.
Ritamarie LoscalzoHOST
5:54
and when we advise people correctly based on that.
Ritamarie LoscalzoHOST
5:57
I'd much rather consider that information alongside, of course, their history and other findings, right? Another thing to look at is HOMA-IR.
Ritamarie LoscalzoHOST
6:06
That's a tool that a lot of people use, and it combines fasting insulin and fasting glucose into a calculated estimate of insulin resistance.
Ritamarie LoscalzoHOST
6:14
And it certainly can be helpful in research and selected clinical assessments, but it has limitations too.
Robert MakiHOST
7:24
which is what we like to use most of the time.
Robert MakiHOST
7:27
And I believe there was a group of resistance training with some cardio of some sort in which is where, and the results that they presented or talked about, which it's called the HOMA-IR score, which is just a classification.
Robert MakiHOST
7:41
It's a calculation.
Robert MakiHOST
7:42
You take your fasting glucose multiplied by your fasting insulin divided by 405s.
Trevor BachmeyerHOST
8:43
Sleep that actually works, it restores insulin sensitivity through cortisol rhythm normalization.
Trevor BachmeyerHOST
8:48
The early detection through fasting insulin and HOMA-IR, not HbA1c, by the way, which is a three-month trend.
Trevor BachmeyerHOST
8:54
It's garbage.
Trevor BachmeyerHOST
8:55
HbA1c is a garbage number.
Trevor BachmeyerHOST
8:56
HOMA-IR, do the math.
Trevor BachmeyerHOST
8:58
None of which that wearable can ever deliver or detect in any capacity, not even slightly.
Trevor BachmeyerHOST
9:03
The disease is molecular.
Ken BerryHOST
4:39
They checked their glucose, their insulin.
Ken BerryHOST
4:41
They calculated their HOMA-IR, which is an indication of how insulin resistant you are, and that's very important.
Ken BerryHOST
4:47
for PCOS.
Ken BerryHOST
4:48
They checked a lipid panel.
Ken BerryHOST
7:36
They checked all their labs again, and they did a DEXA scan again, and they calculated all these numbers after the 12 weeks of doing what they called a ketogenic Mediterranean diet.
Ken BerryHOST
7:47
Every single lab result went in the right direction.
Ken BerryHOST
7:51
Every single hormone, every number on their lipid panel, their insulin level, their blood glucose, their calculated HOMA-IR, their insulin resistance just doing this half-hearted ketogenic diet for 12 weeks.
Ken BerryHOST
8:09
And so that's what they did.
Gil CarvalhoHOST
7:29
So we always have to remind ourselves of that and look at the actual details.
Gil CarvalhoHOST
7:32
A couple other metrics like LDL cholesterol and insulin resistance using the HOMA-IR model.
Gil CarvalhoHOST
7:39
seemed to be improved in all groups or at least trending towards improvement but no significant difference between the groups the investigators also looked at side effects adverse effects of the interventions and the most common one was hypoglycemia so sugar levels being too low this happened in about four to five percent of participants in all three groups so this is one reason i always recommend that people with type 2 diabetes who are doing these interventions either getting on a new drug or doing a substantial dietary regime, changing their diet substantially, always do this.
Gil CarvalhoHOST
8:12
Work with your doctor so that you can keep an eye on these metrics and avoid these hypoglycemic episodes.
Gil CarvalhoHOST
8:36
Now, on that previous video with the other trial, a lot of you guys asked a very intelligent question.
Gil CarvalhoHOST
8:42
is hemoglobin a1c being lower kind of artificial is this happening just because these people are fasting these people are cutting their calories so they're eating less so their glucose is going to be elevated less is that all that's going on or is there an actual metabolic improvement in other words are they still diabetic but their glucose just isn't spiked as much because they're eating less so it's kind of masked or did they actually improve their metabolism so that it's no longer diabetic this is a great question and my answer for this trial is very similar to what it was for the previous trial i suspect there is a metabolic improvement because other metrics other than hemoglobin a1c also corroborate this so fasting glucose for example as we said in the completers in the participants that actually finished the trial fasting glucose was reduced in all three groups and more so on 5-2 so that can't be as easily explained by less eating, less glucose spikes because fasting glucose is more a function of hepatic insulin resistance and how much intra hepatic triglyceride there is and then the results with weight loss also align with this weight loss in all three groups more so on 5-2 so it kind of aligns with this idea that there is an overall metabolic improvement that is delivered by this dietary regime.
Gil CarvalhoHOST
10:03
It's interesting that HOMA-IR was not significantly different between the three groups.
Gil CarvalhoHOST
10:07
So looking at kind of a general measure of insulin resistance.
Sten EkbergSOUNDBITE_SPEAKER
6:15
So low carb generally in the low carb community is 50 to 100 grams, which would give you 10 to 20% of calories from carbohydrates.
Sten EkbergSOUNDBITE_SPEAKER
6:26
so keto versus low carb versus high carb is a continuum and depending on your situation your genetics your lifestyle your history you're going to be at a different place a different place is going to be appropriate for you so anywhere from two percent carbs on the low end up to maybe fifty percent carb could be appropriate for a certain person and it depends on how insulin resistant are you so while there's a carb continuum there's an insulin resistance continuum and we measure this with something called HOMA-IR homeostatic model assessment of insulin resistance and what it does it takes your fasting glucose it multiplies it by your fasting insulin it divides by a constant and you get a score a HOMA-IR score and an optimum value is one plus or minus a few tenths so if you're in that range you have a very very good balance between insulin resistance and insulin sensitivity so normal value could be a fasting glucose of a hundred and an insulin level of four divided by a constant of four or five gives you approximately one Once you get up to 1.5, you're still okay, but you're starting to develop slight insulin resistance.
Sten EkbergSOUNDBITE_SPEAKER
7:54
And by the time you get up to two or three or four, you start having moderate to severe insulin resistance.
Sten EkbergSOUNDBITE_SPEAKER
8:01
At the high end of the scale, we could have people like diabetics that are untreated, meaning they haven't changed anything in their lifestyle.
Sten EkbergSOUNDBITE_SPEAKER
9:22
So you're going to have to play around with that.
Sten EkbergSOUNDBITE_SPEAKER
9:24
And what happens if you're too lean, then you don't have enough reserves.
Sten EkbergSOUNDBITE_SPEAKER
9:31
If you get a sickness if you get a cold or a flu then you don't have a lot of reserves and you also don't have a lot of fat soluble vitamins to help your immune system so there is such a thing as too lean and too insulin sensitive so the homa ir is an excellent way of assessing insulin resistance but it doesn't tell us the whole picture so let's look at a couple of examples here If we take a diabetic and let's say they wake up with a fasting glucose of 200 and we measure their insulin and it's 20, we compute the score and their HOMA-IR is 10, meaning very insulin resistant.
Sten EkbergSOUNDBITE_SPEAKER
10:11
And there's no surprise there because every one of those markers tell us that they're insulin resistant.
Mohammed K. PaikaHOST
14:21
First, fasting insulin with fasting glucose.
Mohammed K. PaikaHOST
14:24
Together, they give you HOMA-IR, an estimate of insulin resistance.
Mohammed K. PaikaHOST
14:28
Many clinicians flag values above about 2.5, though cutoffs vary.
Mohammed K. PaikaHOST
14:33
If it's failing, the storage switch is turning down.
Mohammed K. PaikaHOST
16:39
So when the scale slows, check these instead.
Mohammed K. PaikaHOST
16:42
Is your waist shrinking? For women, 35 inches or more signals metabolic risk.
Mohammed K. PaikaHOST
16:49
Are your periods more regular? Are your triglycerides, hemoglobin A1c and HOMA-IR improving? Are you getting stronger? Your clothes and waist often change before the scale does.
Mohammed K. PaikaHOST
17:02
Those aren't consolation prizes.
