Fluconazole

You know, you see babies whose mouths were terribly inflamed.

And then certainly during the HIV era, when the pandemic was really severe, I prescribed a lot of Diflucan.

Right, a lot of antifungals.

You have to

put people on Diflucan for years, right, because this could be so devastating.

Indeed.

So we're approaching a little better understanding of how the immune system is good because it protects us from danger, but it also can be our foe.

With antifungal use?

Fluconazole is the one where clearance is higher during renal placement therapy because the renal tubular reabsorption is not happening.

So in that scenario, we give a higher dose of 800 milligrams of fluconazole instead of the traditional 400 milligrams once a day of fluconazole.

Those are the scenarios where it's higher.

The azoles include drugs such as fluconazole.

They interfere with the production of ergosterol, an important component of fungal cell membranes.

Fluconazole has been used extensively against Candida infections because it's generally well-tolerated and can be given orally or intravenously.

Then there are the polyenes, most famously amphotericin B, which also targets fungal membranes, but in a different way.

It binds to ergosterol and disrupts membrane function.

But the second I got into the adult business and I was around the girls and we talked and were more open because that's what we talked.

I didn't even know Diflucan existed.

Really? Yeah.

Yeah, because every time I took antibiotics, I'd have to go get the over-the-counter.

They will give it to you, but you have to ask.

You do, yeah.

In the last 20-something years, since I've learned about it, every time I've gone to a doctor and they've given me antibiotics, I say, hey, can you give me a script for Diflucan? Oh, yeah.

Not one has said no. Yeah.

So back to the treatments.

So prescription, are you using Nystatin or Fluconazole?

I typically start with something stronger because again, most of the patients coming to me really need a more systemic antifungal.

So I'll typically start with Diflucan or Fluconazole.

Sometimes I need to go to stronger ones because some of these patients have been on that already and are resistant to it.

So I'll use boronox or itraconazole.

And yeah, treatment with itraconazole can work great for these patients.

Fluconazole is also an option.

Or you can switch to a different biologic if that's what's going on.

It seems like the IL-13s alone, like ADRI and EBGLAS, do better in patients with head and neck dermatitis than just dupixent goes.

Fiber supports gut health.

And if you have toxic mold illness, the most important thing that you need to do before you go after the kill or the detox, whether you're taking Diflucan or Sporinox, or you're taking other things like Biocidin or some other products, make sure your drainage pathways are open and functioning.

You're pooping, peeing, and sweating.

Because if you go after the kill before you've opened up your drainage pathways, what's going to happen? You're going to put a kink in your hose and you're going to get very, very, very sick.

Treatment so we just want to address or kill off the yeast in the gut.

That's gonna be really important It's going to stem from there and that yeast may spread systemically to the rest of the body like we discussed So after we finish this first bottle of nystatin, then we come in with one week of fluconazole so you may be familiar with that as diflucan, but the generic name for it is fluconazole and Fluconazole is used for yeast overgrowth of many types, but it is also used in this regimen to come on the back end of this and address any yeast that has made its way into the bloodstream.

If we go back and we remember what we talked about a few minutes ago, we talked about the connection between a leaky gut and food sensitivities.

A lot of times I'll see folks that have been avoiding certain foods for years because it showed up on that food sensitivity test.
