Fat transfer

Anyone who's considering contemplating breast augmentation has heard of things like BII, explantation, capsulectomy, and these are terms that are very confusing.

Breast lifts, fat transfer.

Well, we're going to address all of that today, but this time through the eyes and the experience of our patient Raina so welcome to the show Raina and thank you for coming we appreciate it it's uh i'm very excited that you're here because and grateful because shows don't exist without you i mean literally anyone who knows anything about me knows that i can just talk about anything at any time true but that's not as exciting as having this banter and dialogue not only because you're interesting to talk to but you had this experience and through your willingness and vulnerability we're going to discover a lot of information and I think people are going to really appreciate it.

So let's go ahead and start with your journey.

AKA removing the excess skin.

And the volume part can only really be addressed by adding volume.

We're going to forget about fat transfer because everyone loves the concept.

You're not even a candidate because you're so lean.

Once you get correction, it lasts a really long time.

The other thing I like doing, if I'm gonna be doing anything surgically, is to do some fat grafting.

Hmm.

Um, I find that if I'm already gonna be in the operating room for something, um, it's really easy to tuck a little bit of fat there.

Um, so it's a challenge.

And it, and I think the issue, you know, I'm not sure.

When I've done fat grafting to temples, I don't like it because typically I inject that very superficial, right underneath the skin, and that skin's so thin, there's not a fatty layer there-

Mm-hmm

It's just that Any procedure, given the benefits and risks, it's important to weigh them.

And I feel like fat transfer, which also has its own complications and risks, is far safer and a better alternative to an implant.

Okay, but I heard that fat transfers, assuming women have enough fat to transfer to have the size breasts that they want, it doesn't stay.

It doesn't hold.

It doesn't stay.

So there's a lot of truth to some of these myths, but there's more nuance, which is, yes, not all of the fat that is transferred survives.

It's a range from anywhere about 50 to 80%.

But done in the right skilled hands, somebody who does a lot of fat transfer, who knows how to treat the fat and uses a specific protocol to maximize the survival of the fat, it's actually on par, if not better, than using an implant.

They're an augmentation option.

You mentioned the mesh, but right now people are really into fat transfer and you have ala clay out there with the cadaver fat.

And so is that a viable way to get augmentation or not? So fat transfer, okay, it

totally works.

It's good, but But there are situations where it just doesn't do what people want it to do.

Don't get it on the whites of your eyes, please, but put it on the skin for sure.

The other thing we didn't really hit on was fat grafting.

Now, fat grafting is wonderful and it works very, very well for volume loss.

So you take a little bit of fat from one area and put it in another one.

It has to be mixed up very, very into very, very fine particles.

Yep, yeah, because you can use fat for more areas than directly under the eye.

I don't love to do – I do tear trough, and that's fine, but I don't do a ton of other fat grafting to the lids.

shape? Yeah, I mean, we can do a lot as surgeons.

I would say when people think of a BBL, they think of fat transfer, that you take fat from somewhere in the body and you put it into the butt and hips.

For

the record, I have trademarked the face BBL.

The results from BBL are 80% really, really good liposuction and 20% actual transfer of fat.

Let that soak in.

The results from a BBL or skinny BBL, however you want to think of it, are 80% from really, really good intentional high-definition expert liposuction and 20% from the fat transfer.

Okay, interesting.

Well, because it's subtle and it encourages your body to regenerate its own fat.

So it's not a fat transfer, but it really serves as a scaffold for your own fat to grow in and around the product itself.

Okay, so there's not going to be like any dramatic before and after, if you will?

No.

And that's really what we're all about, right?

That's, you know, this is all, this is great.

And then also going into, um, you know, fat transfer craftsmanship, you know, you know, fat transfer is, you know, it is a whole different level.

It is, and there's many different types of fat transfer.

Well, because it's subtle and it encourages your body to regenerate its own fat.

So it's not a fat transfer, but it really serves as a scaffold for your own fat to grow in and around the product itself.

Okay, so there's not gonna be like any dramatic before and after, if you will?
