Patient records, that is.

That's how many are now sitting inside Epic.

And as of September 1st, clinicians can pull them directly into ChatGPT.

OpenAI just integrated with the largest EHR in the country.

It is ChatGPT who has just entered.

All right.

So on September 1st, two weeks ago, OpenAI launched ChatGPT for healthcare with a direct integration into Epic EHRs.

Clinicians can now pull visit notes, lab results, medications, specialist records, etc.

So in general, ambient documentation is becoming commonplace.

According to Epic, two-thirds of U.S. hospitals on the Epic EHR were already using some form of ambient AI documentation as of 2025.

And the last category or bucket is predictive staffing and scheduling.

These tools adjust staffing in real time based on patient acuity and census trends, specifically to cut overtime and agency labor costs, which is where a lot of the real money is bleeding out of hospital budgets.

But transplant registries do not capture pre-listing stages.

And leveraging the new data that we've acquired, like national data from Epic Cosmos database, for, for, for the very first time, we were able to look at the national landscape, whatever is happening prior to listing.

And again, we, we knew from anecdores there is a lot of attrition.

People, you know, do not follow up.

So that is a-- it was a huge gap, uh, in the kind of a national landscape.

And I think that's a great segue into sort of our next question.

So I think a lot of our listeners will be familiar with Epic, but may not know this Epic Cosmos database.

Can you tell us a little bit about Cosmos?

I think one of our initial challenges is electronic medical records.

Even in today's modern world, we're on the Epic system now, which is nice for people who, you know, all three health systems here are on Epic.

Um, But we try and, you know, make sure that, you know, we can communicate with, you know, our providers, our collaborating practices, so that hopefully if you have a need, you can call, you can speak to who's working.

And so it can be as high tech as the electronic medical record, talking, you know, to practices to, you know, just get on the phone and saying, you know, here's what I think.

And John has a tremendous amount of experience in healthcare tech.

from Epic Systems, executive in residence at 8VC, and then his experience at Digital Diagnostics.

He understands both the FDA clearance side and the reimbursement side, which gives him a unique perspective in this area.

John, I want to talk to you about AI today, but I really want to thank you for joining the show.

Great.

So I just want a little bit on your background.

You were at Epic Systems for a number of years.

Can you just talk a little bit? People are always interested in how people got where they went.

And most hospitals right now are using ambient AI for dictations.

to do your dictation of your operative node, or even to get your data from your EHR, your Epic EHR.

So we already are starting to use some of these tools.

I think within our specialty is where we can, you know, Start thinking, how do we simplify our tasks, you know, as surgeons? You know, I mean, can I just have an ambient AI take a history and physical and within, you know, five minutes, you know, generate, you know, history, you know, and physical that I don't have to physically dictate? Or can I have an ambient AI write my operative report after I've just finished the operation by, you know, by the camera watching it? So we need to identify what our pain points are.

And extremely messy.

And probably dominated on the technical side by one player, Epic.

And Epic has been guarding the systems for a very long period of time.

I think they have what, their revenue is what, like few tens of billions? Approaching

10 billion, yeah.

business.

Approaching 10 billion, completely privately owned, no external investor.

And it's a rumor that any CIO that gets hired in large hospitals in the US is through a recommendation by Epic.
