report.34-year-old Hugo Flint-Khan was a patient at the Newham Centre for Mental Health in East London when he was attacked in the early hours of January the third, twenty twenty-three.

The nurse in charge wrote that Hugo was in bed asleep when he was dead.

And how do we know that in the last set of observations that have been taken for any patient in this country, they are even alive?

The East London NHS Foundation Trust, which runs the Newham Centre, told the coroner it had addressed some of the issues raised and is investigating staff whose failings have been identified.

The coroner has also referred four nurses to their regulatory body and called for the police to look at the case again.

Yes, they've, they've just in, in the last, uh, twenty minutes or so issued a statement.

And the, uh, chief medical officer of the East London NHS Foundation Trust, which runs the Newham Mental Health Center, says that, uh, they are all deeply sorry for the failings that occurred, um, in H-Hugo's care, that it fell well below the standards that they want to provide.

They also say-- agree that it-- with the coroner that it's wholly unacceptable, um- The, the way in which staff acted and doesn't reflect the standards, values, or behaviors that they expect.
