And so, so is it like side effect that keeps us from jumping or is it lack of data that keeps us from starting with dutasteride?
Jonny HatchHOST
11:14
I think it's lackadavida and it doesn't have the FDA indication.
Jonny HatchHOST
11:17
Dutasteride doesn't have the FDA indication while finasteride does.
Jonny HatchHOST
11:21
I see.
Jonny HatchHOST
11:21
All right.
Max PlasenciaHOST
19:36
No point in getting on minoxidil, detasteride, or finasteride for this area.
Max PlasenciaHOST
19:41
However, if it's the crown of your hair where most of the DHT receptors live, it is beneficial because you take Dutasteride and it'll block 90% of the DHT hormone.
Max PlasenciaHOST
19:52
All right, you're winning the battle.
Max PlasenciaHOST
19:54
Then you add Minoxidil, which basically increases blood flow.
Max PlasenciaHOST
19:58
That's all it does.
Max PlasenciaHOST
19:59
So while Dutasteride eliminates DHT, Minoxidil increases the blood flow, therefore providing more nutrients to the hair follicles, which allows it to grow.
Max PlasenciaHOST
20:13
One drawback of Minoxidil is obviously if you stop using it, well, you get that diminished blood flow.
Max PlasenciaHOST
20:20
So over time, you'll probably end up losing the hair.
Mike HartHOST
7:36
I don't think it's necessarily a bad thing to do overall.
Mike HartHOST
7:42
so i think we've hit the monoxidil now pretty good and people should understand uh overall how that particular drug works and again it's not something that's going to help regrow hair and necessarily what it's going to do is extend the antigen phase so it's going to just help your hair grow longer overall and it's going to help it grow a little bit faster so you know an excellent drug overall and definitely one of the most evidence-based drugs for hair loss um let's move on and talk about dht blockers okay so i as you you know may or may not know i'm a huge fan of making sure that you don't block systemic or blood dht so you know i don't like oral finasteride or oral dutasteride for those particular reasons And again, that's because when you block a DHT in the blood, then you're not going to be getting the overall effects of DHT, which are very, very positive for a lot of things related to testosterone.
Mike HartHOST
8:42
So when we think of testosterone, we often sometimes don't think of DHT, but DHT is one that's really going to, help with your overall mood and your drive your sexual function and so that's why some people who take oral dht blockers like finasteride and also dutasteride do get some negative side effects and this can lead to low mood low libido and then also just kind of a general on feeling of feeling unwell or even depression so you know that's why i try to stay away from those things but unfortunately um you know the reality is is that if you don't have some kind of dht blocking going on you're not going to get the full effects of trying to regrow your hair to the best of your ability so you know what is the best solution to that so the best solution by far is going to be topical dutasteride so oral finasteride is something that is just going to inhibit one of the five alpha ductase enzymes that converts testosterone to dht When you take Dutasteride, you're going to be inhibiting two of the enzymes, so two 5-alpha reductase enzymes that convert testosterone to DHT.
Mike HartHOST
9:56
And the thing about topical Dutasteride is that it only lowers blood in the DHT by about 9% to 11% overall.
Mike HartHOST
10:05
Essentially what's happening is you're lowering DHT in the scalp, but you're not getting much of a lowering DHT effect in the blood.
Mike HartHOST
10:13
And that's why I like it so much.
Mike HartHOST
10:56
Whereas with oral dutasteride and finasteride, you're dramatically lowering DHT in the blood, like 70% up to 99%.
Mike HartHOST
11:04
And then with topical finasteride, you're also lowering DHT in the blood by about 30% to 35%.
Caitriona RyanGUEST
3:51
It comes in a topical or an oral form.
Caitriona RyanGUEST
3:53
And then we have drugs like Finasteride, Dutasteride and Spironolactone.
Caitriona RyanGUEST
3:58
We use all of them.
Caitriona RyanGUEST
3:59
So there's lots of stuff out there to help.
Debra DurstHOST
4:14
That's an alpha blocker that can relax, relax the prostate and bladder outlet medications.
Debra DurstHOST
4:19
Like we mentioned before, Finasteride, Dutasteride, which can decrease the DHT conversion, which can gradually decrease the prostate size.
Debra DurstHOST
4:27
in appropriately selected men.
Debra DurstHOST
4:29
So there's lots of different medication options you can discuss with your doctor, but what about supplements? So one supplement that we talk a lot about and one to at least explore is Pygeum Africanum, otherwise known as Pygeum, but it's actually an African tree bark extract.
Paul DiMuzioGUEST
44:56
Sure.
Paul DiMuzioGUEST
44:56
I was, did we already say, I want to say, did we, I'm not sure if your listeners know, we do also have a foreign one with Dutasteride.
Paul DiMuzioGUEST
45:02
So we, and we, and it's more expensive.
Paul DiMuzioGUEST
45:04
So us telling you that it's not necessary for everyone is because that's just the medical reality.
Matt ZirwasHOST
21:13
But the people taking it in the morning had higher rates of concomitant oral dutasteride and platelet-rich plasma.
Matt ZirwasHOST
21:26
So those could have been like... uh, factors, but they said that, you know, they took, took into account in their multi whatever, and it was still, uh, statistically significant.
Matt ZirwasHOST
21:42
Uh, and the, let's see here, the morning people also were more likely to be on, uh, the oral Dutaster, uh, so Dutasteride, I was looking at spironolactone Dutasteride, I got it backwards.
Matt ZirwasHOST
21:57
Yes.
Matt ZirwasHOST
21:58
So Dutasteride and PRP.
Matt ZirwasHOST
22:00
So, but it's the kind of thing that, like, when the patient comes in and they're like, you know, oh, I've got hair loss and the whatever, and they're like, oh, low-dose oral minoxidil.
Matt ZirwasHOST
22:11
And they're like, they could have told me that on hers or him's.
LizGUEST
8:21
That's why it's very important.
LizGUEST
8:23
Nowadays, there's also another drug, Dutasteride.
LizGUEST
8:27
It's from the same family.
LizGUEST
8:29
It does have side effects, but they only happen in 1% of the patients.
Braden Eric PetersGUEST
19:45
Yeah, yeah.
Braden Eric PetersGUEST
19:46
So that would be Dutasteride and Minoxidil.
Braden Eric PetersGUEST
19:48
Absolutely.
Braden Eric PetersGUEST
19:49
What's the first thing? I know Minoxidil.
Braden Eric PetersGUEST
19:51
Dutasteride.
Braden Eric PetersGUEST
19:52
It's similar to Finasteride.
Braden Eric PetersGUEST
19:53
It's a little bit- Dutasteride's similar to Finasteride.
Braden Eric PetersGUEST
19:56
Finasteride.
Adam MurphyGUEST
8:16
If they have a larger prostate, over 35 grams, that you can do medications that shrink the prostate.
Adam MurphyGUEST
8:24
Finasteride and Dutasteride are the most commonly prescribed versions of those.
Adam MurphyGUEST
8:29
And they block the same conversion of testosterone to dihydrotestosterone.
Adam MurphyGUEST
8:36
which was responsible for male pattern baldness in Propecia, but it also causes the prostate to grow.
