Delirium was driving cognitive decline on its own, independent of this.

So the second study used the same cohort of, And this found one in seven adults, that's nearly 15%, experienced a sharp progressive decline in memory and thinking after major surgery that kept getting worse over the following years.

There are circumstances in which even the bravest of men experience a hopelessness of utter despair.

In JAMA, Janji and colleagues report the VITAL trial, a pragmatic, open-label randomized trial across 49 United Kingdom National Health Service hospitals that enrolled 2,508 patients aged 50 or older for elective major non-cardiac surgery, randomizing them to maintenance with a propofol infusion or a volatile agent, with all other care left to clinician discretion the primary outcome days alive and at home at thirty days was essentially identical about twenty two and a half days in each arm with an incidence rate ratio of one point zero zero There was no difference at 90 days, no difference in mortality out to 6 months, no difference in quality of recovery at day 3, and no difference in major complications, which occurred in roughly 1 in 8 patients overall.

Delirium was the same in both groups, with about 88% of patients showing no delirium at day three.

What did differ were minor patient-reported symptoms, less thirst, less hoarseness, and less nausea and vomiting with total intravenous anesthesia.

The two cases of certain or probable unintentional awareness both occurred in the intravenous arm.
