We kick off with a really interesting paper in the BJGP titled Association of the NHS Health Check With All-Cause Mortality: A Longitudinal Cohort Study in Primary Care.

A couple of years ago, I got burned on the podcast because I made an assumption.

That was, if we're made to do something in GP, it must be for good reason.

Perhaps you first notice a tiny bright patch, then it grows, then a zigzag pattern develops, then it drifts across a part of your visual field, then it gradually disappears.

And neurologists in the British Journal of General Practice in 2020 emphasize that that gradual evolution of symptoms over more than several minutes is one of the key characteristic clues that suggests migraine aura.

So think migration, evolution, and then resolution.

So migraine aura often has sort of a a story unfolding.

What have you got for us on advice during pregnancy, Lucy?

So for this, we have used a BJGP article from two thousand and fourteen, and there's a lot of overlap from the preconception advice.

So a little bit of a reminder that all patients taking medication or with active disease should be referred to a specialist.

That flares during pregnancy are associated with preterm labor and fetal loss, so these patients should be referred urgently to a gastroenterologist.

Let's move on to period pain now.

Period pain is a significant problem for teenage girls, but sadly they can be reluctant or uncertain about seeking medical advice.

A useful practice pointer in the BJGP in 2024 considered a primary care approach to the problem.

Menstrual pain can occur with or without underlying pelvic pathology, and teenagers may present with non-cyclical symptoms, making pathology harder to spot.
