A flight instructor was teaching a student how to fly a Bell 206 chopper, and very quickly you can see the helicopter lifts off, and then it seems to lose control very quickly.

The NTSB says the chopper hit the ground and quickly began to spin out of control.

It rotated to the right, then it slammed to the ground, causing the tail boom and the main rotor system to separate from the chopper.
