The latest evidence has also challenged another increasingly popular idea that whole blood is necessarily better than component therapy.

So this year, in 2026, the SWIFT trial involving 10 UK air ambulance services found that pre-hospital whole blood was not superior to standard component therapy for reducing death or massive transfusion within 24 hours.

And a separate randomized trial of pre-hospital type O blood similarly found that no significant reduction in 30-day mortality compared with component therapy.

But there is no answers in all this evidence.

If we go back to those first papers that came out from the work at London's Air Ambulance and then a couple of other big studies in America and the German trauma registry, most people still quoted at that time an ATC rate, so an acute traumatic co-regulatory rate, of about 25%.

And associated with that was a four times greater mortality.

Ben, it's been a big few weeks since last we recorded the podcast.

We've been off to CAA, a lot of really important messages there, lots to learn, lots to talk about with the other ambulance services and both recovering from the lurgy as well.

So we did speak briefly about equipment a little earlier, but some other things that we've been looking at through our various committees, but in particular the Clinical Technology and Equipment Committee.

First of all, some of you will be working in locations across Ambulance Victoria where you will deploy lightweight backpacks to locations where you simply can't carry the existing ambulance gear.

Now, there are separate processes for determination of who should and shouldn't respond in those environments, and that's not what I'm speaking on now.

Yeah, and I think a lot of things in that area are being missed and causing problems and costs down the line.

Yeah, just picking up on, while we're on health, I just picked up from our friends, the Guernsey Press, in the last couple of days, that an air ambulance provider, so the current air ambulance provider, are doing a legal challenge on the new one.

A lot of our contemporaries and friends, there are lots of people who are big Michael Bay fans.

They love him.

And particularly, like, they'll – like, they love – There's a lot of support for Ambulance, right? Which wasn't as big of a blockbuster as some of his other films.

But just generally, there's just love for the guy.

Ryan Gosling has kind of, uh, become more of a premier name, whereas like Jake Gyllenhaal will be in there.
