466: The Evolution of Cartilage Rehab
S
55:54Snay PatelGUEST
I think the sweet spot I've seen with time is usually around the four-and-a-half, five-month mark.
S
56:05Snay PatelGUEST
We have an Alter-G, so we can start a little bit earlier.
S
56:09Snay PatelGUEST
With strength, I like to see at least somewhere around a 65% LSI and 175% torque.
S
56:15Snay PatelGUEST
I feel like if they're there, their quad has enough... strength in it to support the knee yeah and one of the big functional things i do is like you need to be able to do a good eccentric eight inch step down so i know that you have eccentrics in the mode as well so i think those are my big things for like running just to be like hey listen this is this is where we're going to start from but i try to educate my cartilage patients like uh listen i'm fine with you running i'm totally cool with it If you're going to come to me and be like, oh, I'm going to be a marathon runner.
WHY DO SPURS GET SO MANY INJURIES? | EXPERT REVEALS WHATS GOING ON
G
62:24Gary OelinkGUEST
You've only got to look at recent posts by Xavi.
G
62:27Gary OelinkGUEST
He's running on a treadmill called an Alter-G at the training ground, which is basically an anti-gravity treadmill, which means that he can run deloaded.
G
62:36Gary OelinkGUEST
So he can remove, say he weighs 75 kilograms, he can set the anti-gravity treadmill so that he's only running on a 40 kilogram load through his knee so it's like a it inflates a big airbag around him and and when you look at it that if he's not fully loading the knee on a treadmill he's a long long way away from getting out on the grass um And look, I think Madison was fairly quick.
G
63:08Gary OelinkGUEST
Dragason was fairly quick in terms of their ACL recovery.
"I'd Spend A Million Dollars To Get Six Seconds Better At The 800": Riley Witt Of Bicarb 3.0 On Hot Baths That Rival Microdosed EPO, Threshold Reps At 4:00 Mile Pace & Other Science Truths
R
41:22Riley WittGUEST
One of the first, um, people to do this publicly was, I mean, you and I both know Connor Burns.
R
41:28Riley WittGUEST
Um, he worked with his dad, I think he's like a sales rep for Booster, AlterG or something, I don't really know.
D
41:36Dominic SchlueterHOST
Mm-hmm.
R
41:36Riley WittGUEST
And they were doing a lot of work on an assisted treadmill in high school.
R
42:26Riley WittGUEST
Most people, that would be around four if you're more aerobic, you're a 5K, 10K guy.
R
42:30Riley WittGUEST
I'm an 800-meter guy, so my threshold is a little bit higher.
R
42:34Riley WittGUEST
Um, so if I'm doing some reps at four flat pace, and we're, we're working on a theory, I'm gonna do some more testing with this, um, especially if I continue training and, you know, push for something much faster, um, doing some assisted running, that would be, i.e., taking bicarbonate, i.e., taking, um, y- you know, using a lever, using an AlterG, an assisted running piece to basically take some weight off, so I'm running at 70% of my body weight or something like that, um, titrating that up and down to get to the threshold mark.
R
43:09Riley WittGUEST
Um, and then, you know, changing the, the duration as well to make sure that that works.
Achilles Tendon Pain Came Back: What Should A Runner Do Next?

Mm-hmm.
C
4:56Christopher SeglerHOST
"I'm gonna run on an AlterG treadmill.
C
4:59Christopher SeglerHOST
I'm gonna change these shoes," and then you suddenly get worse.
C
5:02Christopher SeglerHOST
Well, which thing caused it? And when you're getting better, the, you have the exact same problem.
S
7:00speaker_1GUEST
... more structured, so.
C
7:01Christopher SeglerHOST
Yeah, I don't ever use the half boots, you know? And, and so yeah, so I, I think...
C
7:06Christopher SeglerHOST
So it's, it's very interesting because it's interesting you use the term structured, because when I lecture at conferences on these things, doctors want a very structured, "Okay, tell me how many days on the wheely thing? How many days of this? How many days at what percentage in the AlterG?" And I never do it that way.
C
7:22Christopher SeglerHOST
And so the thing from the strategy call that will, I think is gonna help you the most right now is this.
Endurance Is More Than Not Stopping with Ultra Runner Mike Wardian
M
27:39Mike WardianGUEST
I think, uh, I got hurt in 2012, and I wasn't able to run on land for four months.
M
27:48Mike WardianGUEST
Uh, but I was, like, running in the water and, like, on an AlterG treadmill.
M
27:53Mike WardianGUEST
So yeah, it'll be definitely, uh, the longest I haven't run, but, like, I might do...
M
27:58Mike WardianGUEST
Have you seen the guy that, like, does, like, a 5K, like, on a refrigerator and stuff? Like, I might be the-
🔋 Marathon Fueling, Strava Struggles, Rachel's Injury Frustrations + Getting Chicked By A Stroller?
R
27:08Rachel DaDamioHOST
I am familiar with it, but I haven't used that either.
M
27:15Megan Connolly MorantHOST
Okay, because it's different than an Alter-G, but the same concept, right? You can take part of the load off, and you could order one online right now and use it on a treadmill at home or at your gym or whatever.
M
27:27Megan Connolly MorantHOST
It might be a little weird using it at your gym, but you could certainly do it.
M
27:30Megan Connolly MorantHOST
So access to a lever is probably... easier than like yeah driving an hour to get but um a lever is another another option where you can run but it's still i mean i get it it's not going to replace the the chatty miles with your friends you know well
Legal Ways To Run Faster with 1:45 800m Runner | Riley Witt
B
46:40Bill EvansHOST
Even the live high, train low 20, 30 years ago, that was a crazy idea, right? And now it's like just regularly accepted.
B
46:46Bill EvansHOST
So one of Riley and I's crazy ideas that I would love to try if I had the time is not just to train with bicarb, but to actually use something to unload or unweight yourself like a lever or an Alter-G and and then run at much, much faster paces that are most similar to the pace you're actually going to run at during a race.
B
47:04Bill EvansHOST
And some would say, well, that's not going to work because you actually need the forces involved with that higher running speed.
B
47:09Bill EvansHOST
And I do agree with that to some extent, but I do think at the same time that if you're obtaining a certain level of metabolic loading, and if you've still involved that level of high intensity loading elsewhere, you probably would see, my theory is that this would work.

Um, and Kittle, man, he- anytime he's injured, he takes his rehab extremely seriously.
J
13:29Jeff MuellerGUEST
He bought an AlterG for his barn in Nashville.
J
13:32Jeff MuellerGUEST
I would pay money to go to his gym, um, out in Nashville.
J
13:36Jeff MuellerGUEST
He, he has the perfect setup, and he took his rehab seriously, as we all was- would expect.
8 Stress Fractures to Kona: Grace Thek on Load Management, Training, and Recovery
G
38:15Grace ThekGUEST
At that point, I was running one to two times a day, and the volume was a lot higher.
G
38:21Grace ThekGUEST
With the load management now, um, when it comes to running, I've also started using a Lever running unit, which is similar to an anti-gravity, uh, running machine, like an AlterG, and it's a portable one, so it fits in my backpack.
G
38:35Grace ThekGUEST
I travel with it, and so if I feel like, you know, a little niggle coming, niggle coming along or feel like maybe I'm pushing the limit a little bit, I'll do an easier run on that to kinda offload the body, and you can run up to, I think, I think it's like 20 pounds off your body weight.
R
38:53Randall CooperHOST
How does that work? I can't, I...
R
38:55Randall CooperHOST
How do you fit an AlterG in your backpack?
G
38:57Grace ThekGUEST
Yeah, it's incredible.
G
38:59Grace ThekGUEST
You should go and check 'em out.
R
39:46Randall CooperHOST

So he's heading, turning in the right direction.
P

He's now on our Alter-G treadmills.
P
6:08Phil HaywardGUEST
He's running with decreased body weight on that treadmill.
P
6:12Phil HaywardGUEST
He'll do that for the next few days and we'll get him back outside next week.
